PEMBROKE YOUTH WRESTLING INVITATIONAL TOURNAMENT Wednesday, February 20, 2019 (During School Vacation Week) Weigh-ins 8-9 a.m. - Wrestling will begin between 9:30 a.m. & 10 a.m. Location: Pembroke High School, 80 Learning Lane, Pembroke, Massachusetts Folk Style – 1-1-1 periods KAP/Flo Wrestling Event – paperless w/monitors South Coast Championship Trophy will be awarded to league team with most points Medals for 1 st through 3rd Place Pembroke Wrestlers are free of charge but still need to pre-register – No admission fee for spectators or coaches Full Concession Stand – No Food Allowed in Gym Headgear – Mouth Guards for Braces - Required Fingernails and skin will be checked at weigh-ins Pre-Registration Required at $25 per wrestler online $30 at door Flowrestling account is free of charge and parent/guardian must register for the account Questions email paul@accessinv.net All wrestlers must be accompanied by a coach or parent at the tournament and are responsibility for them during the event. Officials should be made aware of relevant medical conditions prior to matches (asthma, etc..). Matches will NOT begin unless coach/parent is in the corner for each athlete. Pembroke Youth Wrestling - Waiver Form (02/20/19) PEMBROKE WRESTLING, INC. 2019 Pembroke Youth Wrestling Invitational Tournament PARENTAL CONSENT RELEASE FROM LIABILITY AND INDEMNITY AGREEMENT We, the undersigned parents/guardians/legal representatives of (Student’s name;) _________________________________a minor, do hereby consent to his/her participation in the Pembroke Youth Wrestling Invitational Tournament and do, on behalf of ourselves/our heirs, our agents and our representatives, and on behalf of (Student’s Name:) _______________________________________ forever RELEASE, acquit, discharge, and covenant to hold harmless, the Town of Pembroke and Pembroke Wrestling, Inc, and its successors, boards, commissions, committees, officers, employees, servants, agents and representatives, of and from any and all claims, demands, actions, causes of action, charges, lawsuits, loss of services, compensation, costs, including without limitation attorney’s fees, damages and/or liability of any kind, in any way arising out of or resulting from, directly or indirectly, all known or unknown personal injuries or property damage or death, which we may now have or hereafter have as the parents/guardians/legal representatives of said minor, as well as any and all claims, demands, actions, causes of action, charges, lawsuits, loss of services, compensation, costs, including without limitation attorney’s fees, damages and/or liability of any kind, which said minor has or hereafter may acquire, either before or after he/ she has reached his/her age of majority, in any way arising out of or resulting from, directly or indirectly, his/her participation in the Pembroke Youth Wrestling voluntary tournament. FURTHERMORE, we, as parents/guardians/legal representatives of said minor, agree to indemnify and hold harmless the Town of Pembroke & Pembroke Wrestling, Inc and its boards, commissions, committees, officers, employees, servants, agents and representatives, against any and all claims for damages, compensation or otherwise on the part of said minor growing out of or resulting from injury to said minor in connection with, directly or indirectly, said minor’s participation in this voluntary youth tournament. We further agree to indemnify, hold harmless, reimburse and make good Pembroke Wrestling and its personnel, for any judgments, costs, including without limitation attorney’s fees, expenses, charges and damages of any kind which Pembroke Wrestling, Inc. and/or its personnel are required to pay as a result of any act or omission of said minor or any parent, guardian or legal representative of said minor with respect to his/her participation in this Tournament. We give our permission for our son/ daughter to participate in the tournament at PEMBROKE HIGH SCHOOL.
